Reminder for QR Code access and printing: KMHC

With proof of vaccination needed both in Kahnawake and in the province of Quebec for non-essential services, many individuals are trying to obtain or download the QR code (electronic proof of vaccination). The Kateri Memorial Hospital Centre says some community members have been showing up asking KMHC to print their codes but the hospital says it doesn’t have the resources to offer that service to the community and suggests you ask a friend or family member for help.

The following is a reminder on how one may access their QR Code to print:
  1. Go to the Quebec.ca website at the following link: https://covid19.quebec.ca/PreuveVaccinale
  2. Enter the information requested and click on the SUBMIT button.
  3. If you are unable to access your QR code through this format and you received your vaccines out of province, please go to the Quebec.ca website at the following link: https://www.quebec.ca/en/health/health-issues/a-z/2019-coronavirus/progress-of-the-covid-19-vaccination/vaccinated-for-covid-19-outside-quebec-using-vaccination-registry and make an appointment at a vaccination center to enter your vaccines into the provincial system. You must bring your paper proof of vaccination to this meeting and an identification (ID).
  4. If you are unable to get your QR code as described in point #1 and you received your vaccines in Kahnawake, then KMHC will be able to register your vaccination information. Please contact 450-638-3930 extension 2295 and leave a detailed message. One of our employees will contact you to provide assistance. You will still need to seek printing elsewhere.

KMHC stresses that it is important to call first at 450-638-3930 ext. 2295 before coming to the hospital.
